Title: Holes on Pepper Leaves

I have noticed that a couple of my pepper plants have very small holes on the leaves.  Upon inspecting them closer, I found a few little teeny maggotty type things - about 3mm long and green.

Does anyone know what they could be and what I can do about them.  (I squished all those I found)

I had these on my peppers ast year, they would eventually glue two leaves together and spin a cocoon, I have no idea what they are.

Squish away.
